Mission Statement

At Med Diagnostics Rehab we have created an environment that empowers our patients with specific, goal-related physical therapy methods to achieve optimal physical health. Patients are progressed considering injury type, prior level of function, and personal needs and goals. Our physical therapists utilize the latest physical therapy research to achieve the safest, most effective treatment results. We promote patient-therapist interaction when planning treatment to encourage self-motivation. Our mission is to provide the community with friendly, safe, and effective therapists to better serve you, the patient!

Billing

The Med Diagnostics Rehab accepts cash and personal checks as well as the following credit cards:

Appointments & Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.

Emergencies

Our office provides on-call service for emergencies. Call 954-942-0927 so that the physician on call can be contacted.

Patient Forms

The Med Diagnostics Rehab provides our patients with office forms for your convenience. We recommend you print and fill these forms prior to your appointment to save time.
